Depending on the flavor of the milkshake, you could do different things. A vanilla milkshake would look extra special if you used real vanilla to make the milkshake, resulting in a white milkshake with the black specks (seeds of the vanilla pod). You can also add an orchid flower, since vanilla DOES come from an orchid.
With a chocolate milkshake, depending on how chocolatey you make your milkshake, you can either add dark chocolate shavings for less chocolatey milkshakes and white chocolate shavings for very chocolatey milkshakes. In Europe, they have a hot chocolate drink with mint leaves called an "After Eight", and it might be good in a milkshake to add crushed peppermint candy to the milkshake before blending it. I'm not really a fan of chocolate and mint, though.
Strawberry milkshakes are always good. I'd drizzle a thick strawberry sauce in the fountain glass before pouring in the drink, and then adding white and dark chocolate shavings on top. A chocolate covered strawberry can also be very dramatic.
I think that to make the milkshake look better, you should go for a new flavor and look. For now, I think that if you are interested in making your own whipped cream (either by hand or in a CO2 canister), you should add some vanillin to it, just to give it some flavor. Adding the vanilla pod seeds to it also gives it the speckled look, which can be very classy. Just experiment with things, but nothing that isn't related to the milkshake, otherwise your milkshake will just look weird.
