It is an expression not a truism. If means that most people if given a choice between doing something good for a family member versus for someone else will usually give preference to the family member. Remember that the next time you are in a boat with a friend's family and the friend has to choose between you or his brother getting the last life jacket.

Blood contains a sludge of cellular components. Too many and detailed to get into. Although microscopic, the size and amount of what it contains makes it thicker. I cringe at the thought of what it would take to muck up my water. However water does have a lot in it, like salt and trace amounts of things that shouldnt be there.
If you're talking about family, blood is not thicker than water. Many by marriage,adopt and step persons would give their life for eachother out of love.

If you're speaking literally, blood has more components than water does.

Figuratively, in most cases, family sticks by you more than people who aren't related to you. But these days, even family tends to backstab you.

Are you kidding? Have you seen blood under the microscope? It is soup, just chock full of stuff.
The least of which is glucose, sugar molecules, also red blood cells, white blood cells, iron bits, macophages, other stuff.
