Check with your insurance company on coverage while moving.

Go to your local post office and set up having your mail forwarded. You may want to have them hold everything other than first class and have a friend pick that up sort and only send what is important or you will have to pay to have the other stuff forwarded.
Contact your current Doctors office and find out what they will need from you to have your records transferred once you find a new doctor.
Do you are anyone in the family take medicine - get renewals on any Rx's that are due, and have your doctors write you new scripts to have filled in the new location when you need them.
Contact your Auto insurance agent and find out how your coverage will work during the transfer and how soon you need to be set up with insurance in the new state.
Do you need to advise utilities (ie. water, phone, or electric to shut off at your current location). Do you want a referral message put on the phone number at the current number.
Have you arranged to have utilities turned on in your name at the new apartment.
Are you packed so that you will have access to what you will need quickly at the new location. _PJ's, meds., sheets for the bed, clean set of cloths, glass & tooth brush.
Contact health insurance and find out what coverage you have out of state and for how long.

uhm, i think that postal and doctors etc, can be found through time. it is nice to have all that in advance but i think you'd have to get to know the area first. to find the nearest (also grade a) doc, post office, super market... it's not as serious as forgetting your id or social. but if it is serious, there's always google earth... look things up by type of business in that area.
