I had a very similar incident happen to our family.
You need to call the police in the town she is residing in. Tell them what is going on and that you need to contact her. They will need to know her birthday and name. The police can look up her address then and will most likely go to her house and tell her to call you. They may not give you her address or phone, but you can ask.

Try the Red Cross, Salvation Army or even the police department.
If it is a real emergency any one of those may be able to help you.
They should be able to locate an exact address from the last 4 digits on the zip code.
Recently I hadn't heard from my daughter-in-law for quite some time.
I knew her name and address but wasn't sure she was still there.
Now what would you do in that situation?
I looked up cab companies in her town. Wrote them a letter explaining the situation, enclosed $20 and ask if a driver would go by that address and tell her to call me.
She called 2 days later and so did the cab company in Missoula, MT.

Contact the Milwaukee Police Department. Call your local police department's non-emergency number. They should be able to give you a number to contact Milwaukee.
They in turn, should be able to run her through LEADS, (Law Enforcement Agencies Data Systems) and the Secretary of State, or department of motor vehicles and obtain her address. If you have other information, such as her social security number, driver's license number, date of birth, height, weight, birthdate, that should also be able to narrow the search.
Also if you have her last known address, provide that as well.
I wish you luck. These things should help though. Be prepared to give your own information as well. While they may not be able to give you her contact information, they can apprise her of the situation and advise her to contact you.

Ask the Salvation Army, Red Cross and police department in that area. Explain to them it is a family emergency and to let her know it is and the nature of that family emergency.
Now if she does not want contact there is nothing you can do about it.
Be sure and recontact them if you do not hear from them soon.

In an emergency you can call your police dept. and explain you have an emergency.They can contact the police in her town with the info you have and contact her.They will get her to call you.
Or,call directly yourself to the Milwaukee police and give them the info.
