Confession for catholics are for healing their soul harmed by their sins.
There is nothing about penance. A human being is the one who choose to do good moral acts or evil moral acts (aka sins) so no penance only a really bad choosing.
Well is recomended go to confession at least in Easter, the holiest days for our faith.

Once a year, unless you have a mortal sin on your soul, in which case, immediately.
Venial sins don't have to be taken to confession ever, but the one time a year thing is so you stay in the habit of being aware of your sins generally.

At least once per year. The day varies...some do it on Shrove Tuesday before Lent begins...others like to do it before they make their Easter Communion (also required once a year) - so - Holy Saturday before the Great Vigil is another popular time.

At least once a year -and in any case you shouldn't take communion if you have committed a mortal sin and you haven't yet confessed it -that would be sacrilege. I can't remember what the consequences of sacrilege are but must be pretty dire. If you die in mortal sin, you go to hell... I don't know where you would be sent if you died in sacrilege.

Say you have been to die and you have no longer any mortal sins on your account, the two for the reason which you have confessed any you have dedicated or for the reason which you have no longer ever dedicated any.... you have nevertheless venial sins (little issues you probably did with out objective) and you may pass to purgatory in case you died with out having confessed. yet once you had, you pass quickly to heaven.

Easter duty, preference for once a month and also ASAP if you need for grevious sin. Always before communion (or other sacrament) if you are guity of a mortal sin
