it appears that generally you need the father's consent, but in a case like yours where you cannot reach him then you may be able to do it by yourself. if you have his contact information particularly his address the court may want to try a certified letter. in montgomery county, it appears the child's father would need to be informed via certified mail and "reasonable steps" taken to locate him and inform him that you are trying to change the child's last name. it appears you would need to go to your County Probate Court to change your child's name. (see - http://www.mcohio.org/revize/montgomery/government/probate/name_change.html ) i am not sure how much of this is true of other ohio counties.

if YOU want to change your child's last name, it will have to be done through the courts, and last time i looked in Ohio, it was over $400 to do this.
i am not sure if the father has to sign anything.

I think that if the father hasn't been in his life for a certain amount of time, you can get a request to change it.

Change your last name to what you had it before if you had changed it. Than adopte your son in order to get his name the same as yours. Thats what my mom wants to do. I want her last name and the one that I have right now so shes going to adopt me so I can have both last names.

**** it honey if your not married your baby should have your last name. esp. if the father isn't in the baby's life. i can't really help you on changing name thing b/c i don't live in ohio.
