Why a 3 week notice? The usual is 2 weeks only. Anyway here are some tips for writing the notice.
Even the most difficult job situation should be left graciously and courteously. This is not the time to tell your boss what you really think of him or to offend your ex co-workers by assigning blame. Even weak bridges should not be burned!
A formal, brief resignation letter should be given to your current boss. Do not discuss any negative reasons for your leaving. Simply say that the new offer provides you with new challenges and opportunities. Your letter and discussion should be positive in all respects.

Anything of any resonable importance should be done via some medium that can be recovered later if needed such as email (from your own personal computer so when you leave you can still recover the email if needed) If you talk to them it is far too easy to simply say that it was never discussed.

Just go up to your supervisor or boss and tell him/her that you are giving your 2 week notice. Be nice and polite and try to always leave a job on good terms, it will help for your next job.
