Well, I can't really tell you any ways of overcoming your fear, but I can tell you that being put to sleep doesn't hurt, and you'll have no idea what's going on in surgery. So look at it this way, it could always be worse. No matter how nervous you may get at the thought of being put to sleep, it absolutely cannot be worse than being awake through the surgery!!!! It's just your appendix. You'll never miss it anyway. Remember, the Dr is doing you a favor. You WANT to be asleep for it, and you WANT to have your appendix out so you don't die. You'll be fine.

Its common to worry about surgery-- being cut open is scary. I have had several surgeries and i was very nervous ahead of time. The docs can prescribe sedatives for you to take before the surgery to calm you down, and by the time you are wheeled into the operating room, you won't be capable of worrying.
When you wake up from the surgery, the incision will be bandaged, and you won't be able to see the incision. You can just look the other direction when the bandages are changed. The incision will probably be a little uncomfortable for a few days, but probably not very painful. Your abdominal muscles will also be a little sore for a few days, but people don't usually have serious pain from this type of surgery.
Dealing with fear of surgery is the same as dealing with any other type of anxiety. You need to accept the fact that surgery makes you feel nervous, and just concentrate on relaxing and breathing slowly and deeply while you feel nervous. Most of the time when people have fears, they end up being more afraid of their reaction to anxiety (rapid breathing, high pulse rate, etc) than the thing causing the anxiety. This web link has some good advice for dealing with anxiety:

The comment about your never waking up was cruel and unncessary. You will be just fine. Appendectomies are among the most commonly done procedures. You'll breeze right through it. After you check in and are taken to the preop area, you'll be given a gown, socks or booties and a cap. You'll be shown to a changing room and asked to undress and put on the things I've mentioned. Once you're in the holding area, you'll be lying down on a gurney. Your surgeon will visit you briefly, your mom can be there. You'll be given a preop sedative and when it takes effect, almost instantaneously, you won't be afraid at all. You will do great.
Afterwards, you'll be waken up by your anesthesiologist and the OR nurses. You'll hear them telling you to "wake up, it's time to wake up now". And you WILL wake up.
You won't bleed. You'll be sore when the anesthetic wears off but you will be given some pain medication and you'll be fine. You'll be up and around in no time!
It's okay to be nervous. That's normal. You'll do just fine.

Unfortunately you sort of have no choice to get this procedure done. That is a truth to face. But to make it easier, be reminded of the other truths..... a) Nobody feels anything during the process. b) You will not wake up or have pain dreams during it. c) The professionals know what they are doing and they know to wait until you are properly under. d) The drug kicks in fast and you will feel lovely and relaxed as it does. e) I repeat, you will NOT wake up. Knowing that pain will not occur, just put yourself in robot numb mode and don't think about it. March on in and tell them to get those problem teeth out of you. You have 2 weeks to scare yourself even more. Email me if you freak out closer to the date. No choice dude.... so weigh out the logic and truths, versus your imaginary fears. And don't forget another truth.... it is well known that when you are put under it feels like you wake up only moments after being put under, without any awareness of the time which has passed. So when you sit in that dentist chair, be reminded that it will all be over within 'minutes'.

Don't worry, when they put you under, you won't feel a thing, I know. I recently had my tubes in my ears taken out. (Weird I know.) And so, they had to do an operation, just take a deep breath whena round the doctor, and ask some questions about what is going to happen. That's what I had to do! Good luck. :D
