I think this is a 110 plug in. If you use an adapter is the appliance still grounded and safe? Also some adapters have a little metal circle on them. What is this for and is this better than the regular plastic ones without it. Thanks for any help!

if the plug has 3 prongs, the round one is ground.
It will be connected to the chassis of whatever it comes from.
If it is not connected to the outlet, for whatever reason, and the Unit develops a short to chassis and you grab it, make sure your insurance is paid up.

All the adapters I have seen have a wire with a terminal on it. That is meant to connect under the screw holding the cover plate on the outlet. Many times, the electrician neglected to attach the ground wire, or if it is a 2 wire outlet, there is no ground.
I haven't seen a metal circle unless you are describing the above.

bottom line without implementing a ground on a 3 wire prong, don't try to litigate.
