My favorite framebuilder by far is Bob Jackson Cycles in Leeds England. They are represented in the US by World Class Cycles in New York. You can do a search for Bob Jackson Cycles and find both websites. Jackson has been in the custom frame business since 1935 and offer some of the most beautiful work I have ever seen.

On top of getting ANYTHING you want as far as frames, they provide your choice of paint jobs- whether it is 1 color or 20- at no extra charge. They also can do chrome plating and all kinds of other stuff.

The best part is they are usually HALF the price of the people who do nothing except weld some tubes together and call it custom. They are artists and craftsmen.

Jeff Jones, if you like mountan bikes, they're a beautifull Titanium bikes.

Titus and Ventana also offer custom sizing on some of their bikes.

It just depends on how much you're willing to pay and what you're looking for.

Two other makers to consider:
CoMotion out of Eugene, OR: Maker of Tandems, Road, Track/Fixed-gear bikes and TimeTrial bikes as well as touring frames. They work in either Aluminum or Steel.

Veloforma out of Portland, OR: Maker of Carbon-fiber racing/track and TimeTrial frames.
