You need an alpine head unit (car stereo) that is compatible with the alpine blue tooth unit. The blue tooth unit from alpine is $200, after you have them installed all you need is to turn on your blue tooth phone, pair the two together and your all set, there will be a small microphone installed by you so they hear you and you will hear them via your cars speakers.

There are a variety of hands-free kits that will allow this with factory radios. For integration with factory speakers, the best kit I've installed was a Parrot: http://www.parrotshopping.com/us/p_parrot_item.aspx?i=138086
The parrot kits include speaker inputs/outputs so they interrupt the factory speaker wires and substitute their own output when you get a call. Other Bluetooth kits, like some Motorola versions, would require extra relays if you wanted to hear the audio through a car speaker.
If you have an after-market deck, or want to replace your factory radio, some new models have Bluetooth capability built in. Others (not just Alpine) offer Bluetooth adapters that can be added on for an additional cost.
For just about any of these devices, the first requirement is a Bluetooth-enabled phone.

Pioneer actually has a new radio out that has Bluetooth built in, without the need for any additional adaptors. Price on the unit is roughly $400. But you get a new radio, with the bluetooth built in. Displays incoming and outgoing calls on the radio, and also mutes the radio when receiving/making calls.
