If I don't know someone well and refer to him / her as Dr. Smith, I will do that all of the time, not only sometimes. Doctor is a title like Mr. It does not change.
I worked for a doctor for several years so knew several of them socially. I referred to them by their first name - I didn't switch back to "doctor' when they were in their work clothes.

For medical practitioners, just consider it an act of cruelty - to remind these poor wretches that they are always on call, that they can never get a moments peace and the they are generally expected to save the day at the drop of a hat.
Sounds a lot like Superman/Superwoman to me

Depends on the nature of your relationship with your doctor. Some will ask you to call them by their first name. I worked in an organization where there were many doctors, we all addressed each other by our first names, doctors or not.

college prof's that are PhD are also called Dr so and so. they must publish papers and books. It is a sign of respect for all the hard work and respect they deserve.Md's that are specialists can 12 or more years or education and never stop learning.They are often published and conduct or are involved with medical studies.

They have earned the title so you must address them by that title, especially if it is a profession. Almost all people in professions take an oath before being admitted into that profession. They have to stay true to that oath at all times. There is no off duty from it.

I've had the same Dr.for 3 yrs and i aways calls him Dr. and all the other Dr. in his office i also call them Dr. the workers and assistants i called them by their first name.

My uncle is a Dr., but I never call him that, I don't even use it when I write him letters.
I guess it depends on your relationship with the person.

Whatever job a person has, they've earned it.
