I don't think they would actually fall off, but if you're running a marathon you'd definitely want to tape over them or put band-aids on them to keep them from getting rubbed raw. I ran a marathon once and saw some pretty nasty raw nipples on a few people.

I suppose in the very extreme it is possible to do serious damage to them, however it is unlikely.
Your clothes gently rub against your skin, if you sweat they rub a bit harder so when you exercise this rubbing is harder than normal. This can cause jogger nipple - your T Shirt or whatever rubs and over time this can be very painfull (if you look after a long distance race - half marathon or more, you might see men with 2 bloody streaky down their t Shirts - a very painfull case there).
So it can feel like they have been rubbed off, and there could be blood too, but they are like grazed and will heal OK, and it can be prevented with sticking plasters or vaseline
You only make the mistake once of not protecting them!!

I have heard that some runners experience nipple bleeding during running I can't remember the exact reason why, but they do not fall off.

A nipple couldn't just fall off. Of course it could be bruised or hurt, but being part of the body, unless it was severed, it won't 'fall off'.

It is not possible. They might become irritated if they are rubbing against your shirt, so some women wear a sports bra or cover them so they do not chafe.
