I had applied for a passport 2 yrs ago from my home town, but was not able to be present for police verification as i had newly joined a job in pune and couldn't get leaves. Now i want to reapply for the passport , but i have lost my file reference number of the previos application, how can i proceed. please let me know asap. thanking in anticipation.

Execute an affidavit of loss for your previous file reference number and submit it together with your application for a new passport.

Please try to follow these steps.
1. If you are not very far from ur Regional Passport Office and you remember the moth & year of ur application then go there and try to check at counter itself regarding the status. They might charge u some amount.
2. As you did not appear for Police verification, the Police might have sent a negative report reg u. If its -ve then they will never ever issue a passport to any one.
3. Try to talk to the RPO and also try to get a Police verification from your local Poloce SP and submit it to them.
4. The format for this verification should be typed on the letter head of the SP ( see Annexure F in the instruction pages of the new passport application form )

simple. file a fresh application. under the heading whether u r holding a passport or applied for a passport give the facts as stated by u. Meet the P R O in passport Office and expalin the circumstances to him/her. The PRO would help u out

If you previously applied for a fresh passport and only couldn't get due to verification failure, Best would be, apply again with fresh application stating the previous conditions in additional statement & declarations.

Know the same from RPO. Your case is not a case of re-issue. This is case of application for fresh passport.

i think i can helkp in that listen 1st n formost dont hide the fact as it wont help,second for applying for a fersh passport ,make an affadavit in your name stating what happened lst time and get it atested by 1st class magistrate ,atach it with other documents ,u havent mentioned where do you reside now or from which regional passport ofice u wish to apply,if possible mail me at
