My question is: Why is it ok to do it once? This seems to be another mormon double standard.
What about my nose?

Probably nothing unless she went and asked her bishop about it or someone she knows asked about it or ratted her out. The Mormons spend a lot of time with their head in the sand unless something smacks them in the face. They don't really want to call attention to anything rebellious or wrong in their religion. This is very different than religions like Jehovah's Witnesses where you can get outcast by the entire congregation relatively easily for wrong doing. I really think I admire the JW's more for the much lower levels of hypocrisy among members. Yes they are whack jobs, but for the most part their youth do not think they can do whatever they want (Drinking, sex, drugs, sex) and then just repent and promise to be good. Plus Mormons are one of the most stubbornly prejudiced groups left in this day and age (predominantly related to race and sex).

The first step with something minor like piercing would be attempting to guilt the parents to fix the situation. Then they would probably just call her in for a meeting with her Bishop, and he would counsel her advising her against the extra piercing and to let it close up. They wouldn't threaten her or anything. They use guilt very masterfully and much more often then to make threats. I knew many kids growing up that SWORE that their Bishops could read their minds when they sat in front of them. They were very paranoid. Most times the guilt would do the trick.

Wow, mormons have seriously gone over board, christians in my church get their nose piersed and come to chruch in jeans and we have some goth and some skate boaders and some punks with differant colors in their hair and some women that need to watch their hemlines, but otherwise, we love the sinner and not the sin.

Mormons believe in avoiding extremes in appearance. That includes piercings, tattoos, hair styles/colors, clothing styles and modesty, grooming and cleanliness.

Their are no Mormon fashion police sneaking around to catch and punish perpetrators. It's kind of a personal thing. You know your religion and you choose what parts to live. Those who believe their religion strongly try their best to live it all. Those who believe partially live it partially.

No ones going to hell for having an extra set of earrings, but you can generally tell how devoted they are to their religion by how much they follow it.

She would live on just fine, her ears may hurt a little for a week or two but heal fine, and the world will keep turning.

Some Mormons even drink sodas!
