James 5:13-16 says:
"Is anyone among you suffering? Let him pray...Is anyone among you sick? Let him call the elders of the church, and let them pray over him, anointing him with oil in the name of The Lord. And the prayer of faith will heal the sick, and the Lord will raise him up. And if he has committed sins, he will be forgiven."
-This is a very spiritual act which cannot be performed by just anybody. Only those(pastors and elders) who truly live their lives according to Christ can be allowed to anoint the sick.
-The oil is taken from a new bottle, dedicated to The Lord and used only for the purposes of anointment.( it should be placed in a small bottle)
-The hand of the pastor and elders are placed on the person who is sick, they are on their knees while they pray. The oil may be placed on the forehead or any where on the body, where the illness is present.
-St. Paul warns about placing your hand on just any body's head.He says, "Do not rush..." Because some people are possessed by evil spirits.
-Some illnesses are caused by sin, and its essential that the person confesses all their sins prior to anointing.
-And according to God's Will, the prayer of faith shall heal the sick.
It is very sad to see how some people carry around a bottle of oil and attempt to anoint every one who crosses their path.
The Lord is present whenever an anointment is taking place and it should be a solemn, spiritual event.
"For where two or three are gathered together in My name, I am there in the midst of them" Matthew 18:20

The Orthodox or Byzantine Rite( and other Eastern Rites,I believe) anoint all the senses in Holy Anointing. The Vat II Roman Catholic anoints the forehead and the palms of the hands.

the blessing or annointing of oil is applied/poured onto the head. This pagan ceremony was in practise long before christianity.

Both....well - I am speaking of two different things.
In the Sacrament of Holy Unction, only the forehead.
During the Last Rites, the forehead, eyes, lips, heart (chest), hands and feet are annointed....

It can be either. You can also annoint a stand-in when a person is too ill to come to church. It is not the oil that heals, it is the prayer.

In the Bible, kings are annointed with oil. Hence, the messiah (annointed) will be King of Israel -- not king of some mythical kingdom in the afterlife.
