Can anybody please check why my form mail isn't working?

I have no idea why.. any help will be greatly appreciated.

This seems like a stupid answer but here goes.
In your form tag your action is set to "".
The submit button has not OnClick function.

It therefore appears as if you are not sending the form anywhere once the details have been completed.
No you could not have made such a simple mistake. I must be wrong.
Like I said I am sorry for such a stupid answer.
