Did McMurphy die at the end, like did the Indian guy kill him? I don't really know what exactly was happening.

Yes, Chief smothered him. He couldn't stand to see such a free soul turned into a vegetable. He knew McMurphy wouldn't want to live his life like that.

They operated on McMurphy's brain so he would calm down and not be so much trouble. He had scars on his forehead.
The Chief killed him because he had lost his independence and the Chief assumed he would probably rather die than live like that. McMurphy taught the Chief how to be free.
The movie is on TV right now.

McMurphy was turned into a vegetable after electro-shock therapy. Chief Bromdem smothered him to put him out of his misery. The Chief escaped afterward.

Yeah . As Chief smothered him He said "I'm taking you with me." McMurphy twitched, fought, then died. Then Chief smashed the window open with the sink and leaped through the window and ran off into the woods.
