Worse case? Maybe it's you don't fix it, you just keep on driving it until the wheel comes off while you are going down the highway, this throws you into the path of an oncoming semi and your decendants spend the rest of their life paying the liability claims from the forty other cars that were involved.

Worst case it could ruin the hub and wheel spindle and obviously all the bearings which could get very pricey. As far as driving the vehicle I have seen vehicles with wheels that come off due to bad wheel bearings or at least get so noisy that you are afraid to drive the car.

The first thing that hopefully will happen is it'll start groaning or squealing. Then, it'll either seize up and the wheel will cease to turn or the bearing will literally fall apart, your wheel will wobble violently, cut into the axle (or spindle), ruin it as well and the wheel will eventually fall off.

Best thing - get it replaced ASAP once you determine that's what it is. Next best thing - replace it immediately once it starts making noise.

Well for one you can have a noise emitted from the bearing. Depending on how bad it is and how much play is in it, you could have some abnormal tire wear. Also you may fail your next safety inspection depending on your state's requirements for safety inspections. One other thing is it may affect the braking system depending on what type of brake syytem is on the affected wheel.

the weel could fall off if the bearing starts to break up and cut the axel
