We assume you mean the plug, as the jack is the female receptacle.

Cut the wires off as close as possible to the jack.

Carefully strip back the two wires, exposing about 1/2 inch of bare wire.

Then add an end like this, for 1/8 inch size. (standard on ipod and most home headphones.

If the Headphones are that good that you need to save them, the best idea is to cut the able about 2-3 inches back from the jack and 1. solder a stereo Hobby- jack, (D.I.Y) to the cables, (remember that both terminals share the same earth!!..ie. 3 connection points , 1 left channel, 1 right and one Earth (-)terminal) .
2, bare all the cables and splice them onto a salvaged jack cable, ( but take care not to damage the insulation on the red and white, each channel keeps it's own earth (-)), ..bind R.earth to R earth and insulate with tape, red to red and insulat e and then isulate the two cables together. repeat the same process to the left channel and then bind all the cables with more tape for extra protection,(and it will look better with the extra tape!!).

Try Maplins, if you have a local store otherwise try their web site. They will sell jacks. All you need to do is cut the old jack off the cable and attach the new one. It is so simple and you don't have to worry about the wires as they can go in any way as audio cables are universal (both wires are both live and neutral)
