Sorry if I confused, for example if the name of a person is,

Ms. Annie Doe-Smith, should it be Dear Ms. Doe-Smith?

you should use both name but if you don't then go by the last name. a lot of women hyphenate their names after getting married and the last name is the married name. I hyphenated my name because I haven't gotten around to changing it downtown.

If it is a business letter, yes. If it is a personal letter, the first name should suffice.

You address the letter the same as any other business letter...Mr Smith....Mr. Smith-Jones....

yes, you address them exactly as you would if they had one last name, you simply put both last names, as you have done.
