As a Mental Health Therapist, to me this person sounds like an Anti-Social Personality with some Borderline features. However, a qualified doctor should do testing to recommend a course of treatment. That said, however, the prognosis for someone with Anti-Social Personality Disorder is extremely poor. These people don't see anything wrong with their behavior even when it gets them into trouble. Borderline is a little better in terms of treatability and generally lessens as the person gets older. This person should get treatment ASAP.

The initial response so far has been that this person exhibits tendencies toward being a sociopath. He is not. He does exhibit many traits attributed to persons who are diagnosed with Borderline Personality Disorder. This is a neurosis and not a psychotic disorder. People who are borderline tend to be very self centered. Narcissism is high on their list of preferred states of being. There is no medication that can help this person; it is a learned behavior and requires an acknowledgment on the part of the person to make a conscious effort to change their behavior. People who display the level of behavior you describe are rarely interested in changing their ways unless they can gain something for themselves in the process. There are varying degrees of this behavior and I would say this person is about mid range. This disorder is not like a drug or alcohol addiction and an intervention cannot be done in an effort to change the behavior. He will have to seek out help and then follow treatment recommendations. For people at his level it is a long grueling process that is usually more painful for the therapist than it is for the patient.

Well, that's a new form of diagnostic routine to me! What kind of a "smell" can a patient give off if they are mentally or emotionally ill? The only time I could imagine a doctor using smell as a diagnostic tool is if the patient is suffering from poor hygiene and neglect of health. This is a symptom of mental illness when people do not bathe or clean themselves. But that's it. Otherwise, this sounds goofy to me. And yes, I would run out of the clinic and report this to the clinic's administration.

Only a professional can diagnose a mental illness, and only after seeing that person for some time. Your friend may be bipolar, schizophrenic, a psychopath, or just plain be a mean SOB. It sounds like he has a future of state ordered mental health care. I would see about getting him help before he gets himself in serious trouble or hurts someone. In some cases, being a stalker can land you on a sex offender list, and that’ll bring a heap of trouble. You may be able to call in an anonymous tip to the police when he is in the middle of a stalking episode and have him arrested for the offence to help wake him up to his problem.

The person is a sociopath.

Depending on how well he can channel it, he will be able to work well and be successful.

However, due to the "extras" you've put in, he will need professional help to avoid ending it at the wrong end of some poor cop's firearm.
