he knows about dreams, but he insists that this was real no matter how much we talk to him. My grandmother's feeling are hurt because he used to call her daily but now he's afraid...what should we do to get him to realize it was not a "vision" as he is calling it, but a dream?

You may to call a prayer line with a church which is anointed and moving in the power of the Holy Spirit. The son may be right in saying that he saw a vision. It sounds like there are demonic spirits which are bothering him. And demonic spirits can give you visions and dreams both. So, the demons are the ones feeding his fear about his grandmother. But yeah, call that prayer line and ask for restoration between your son and his grandmother.

have you considered that he may be telling the truth? you may trust the grandmother but it would be foolish to ignore this.
