Cute parent survey for toddlers in daycare?

I teach the Crawlers class at a daycare. I'm trying to come up with a cute survey for new parents to fill out. So far, I have favorite color, favorite cartoon character, favorite sports teams (college and pro), favorite song, book and their pet's name. Obviously, the kids are too young to care about any of that, but I thought it would be a cute survey for the parents to fill out for them. What are other cute questions I could ask?

Favorite hobby (you'll probably get creative answers there), favorite place to go, fave toy, where/when they were born, siblings, famous quote (if they talk yet), future aspirations, worst enemy, favorite season, an interesting fact about them.

Okay.............

First, favorite "sports teams" is simply asking the parents what sports teams THEY like (your crawlers could care less, and most likely do not even realize if they are watching the Cowboys or the '49ers)

Favorite food (mashed or solid)
Favorite toy
Favorite friend
What makes baby smile or laugh?
What is the biggest mess baby made?
Favorite daily ritual
Favorite nite-nite toy/blanket

Gotta tell ya, when my first child was a toddler, I MIGHT have taken the time to fill out a teacher's survey. When my second one was, it probably wasn't gonna happen. It's not that I cared less -- it's that along with all the other issues in a day, including pick-up, drop-off, dinner (for us and for baby), bathtime, playtime, bedtime (which can be extended for a crawler), laundry, dishes, straightening up the house, getting the 6 hours or so of sleep I was able to get, get up, shower, get ready, get kids ready, and drop them off at daycare, I wasn't going to sit and answer a "cute survey" from my crawler's teacher that asked questions like "what is your baby's favorite sports teams?"
