In anger, Hiroshima and Nagasaki.
For "research"
US - the Nevada test sites and Pacific proving grounds (in the Marshall islands) with a few other sites in the US used, such as Alaska, Colorado, Mississippi, and New Mexico - see http://en.wikipedia.org/wiki/Nuclear_weapons_and_the_United_States for full details
USSR - mostly at Semipalatinsk test site and Novaya Zemlya, with a few taking place elsewhere in the USSR.
France - mostly in the Sahara, in (then) French Algeria and Fangataufa and Moruroa in French Polynesia
UK - mostly at Maralinga and Emu Field in Australia, some at Christmas Island in the Pacific - plus many joint tests with the US.
China - Lop Nur weapons test base in Xinjiang, China.
India - Pokhran, Rajasthan, India
Pakistan - Chagai hills, Baluchistan, Pakistan
North Korea - Hwadae-ri, North Korea
Most of those tests were underground - so they weren't 'dropped' as such.

Hiroshima, Nagasaki, New Mexico and Bikini island as far as I know. The first two were attacks, the second two were tests. Also, North Korea did a small test recently, not sure where it was.

The only country unfortunate enough to be nuked is Japan (Nagasaki and one other city) during the second world war. There have lots of other tests around the world.
