This is a lot of overworking for a little splinter. You can remove it yourself with a pair of tweezers, after soaking your hand. If you can't bring yourself to do it, ask a friend. It's not necessary to visit the doctor unless it seems to be getting infected. He can remove it, and he will not need to use anything fancier than a smaller pair of tweezers. You won't need anesthetic. You probably won't need more than an antibiotic ointment and a bandaid, and you won't need to miss work more than an hour or so if you decide to go to a doctor. Really though, just soak and get it yourself. Or leave it be and your body will force it out. Cover it with a bandaid and leave it be. It will be sore, and red, and puffy, until it gets forced up to the surface. At that point, you can probably pick it out with your fingers. But really, it's not a major injury here. It's a tiny little splinter, not some hunk of wood you impaled your hand on.

Most splinters can be removed at home. Use a sterilized needle and wash the area before and after. After you remove the splinter, use Neosporin and a Band-aid on it.

If you can't get it out yourself or don't want to try, a doctor can take it out for you. He won't use an anesthetic, you won't have to miss work, and the anti-biotic shouldn't make you dizzy. (If it does, call the doc asap.)

A splinter of the small size you describe should not affect the muscles in your hand.

I've had to take out a LOT of splinters from myself.

You don't need to see a doctor for that unless they are in very deep, but you will want to douse the area with alcohol after you pull the splinter out with tweezers to keep infection from setting in.

after awhile, they work their way out of the skin, but I am pretty sure you can get it out yourself but, make sure the wound is clean; it could get infected.
