If you're taught traditionally, two ranks: without Mongkon, with Mongkon. The Mongkon, or headband, that fighters wear to the ring is a sign that they are trusted and respected by their Kru, or trainer, to have a good understanding of Muay Thai and are ready to fight professionally. Basically, when you prove you;'re ready, they give you the mongkon.
Any other rank is bestowed upon you by the fans, ie. by your betting take in house, how popular you are with fanbase, your ring name, and your win-loss record. So, if you win alot and the fans love you, you're near the top. And if you lose alot and the fans hate you, you're near the bottom. Keep in mind, this is all in Muay Thai as traditionally trained, not particularly as trained outside of Thailand. There are some trainers who adhere to these traditions (generally Thai-Americans or Thai natives) and some who do not (generally American/European Muay Thai trainers).

I don't take Muay Thai, but I honestly think that there is no ranking system. I know they wear those arm bands on occasion (Which look pretty sweet to me :P), but I don't think that determines rank.
