You go to a doctor and promise to pay them later. THEN you honor the promise, and get a job.

If it's bad enough and doesn't clear up quickly, go to the emergency room at a local hospital - they have to treat you whether or not you have insurance or money to pay. You'll still get a bill though. If you don't have insurance or much income, they might lower it substantially - talk to their accounting department.
