i had surgery on my big toe, and other places too. the big toe was the worst pain i have ever had. i get crazy in grown toe nails. the best solution is just constant TLC.
First I grin and bear cutting the in grown part out. Then trim the nail just barely - don't cut it all off!
Then over the next couple of weeks I trim only the middle part of the toe to keep tension off the part that will in grow. Eventually I will have to cut the in grown part again, but it is much less painful because there is less tension.
This is the best method to maintain the nail that I have found. But if you have already been whacking at the nail and have found that a "piece is stuck". Then I would have to recommend either toughing it out until it grows long and manageable again or go to a podiatrist. A podiatrist needs the work anyway. I toughed mine out and it was almost a religious experience unequaled by fasting or stigmata.

Easy fix. This happened to me many moons ago (my right big toe) and recently on my middle finger (of all places). Here's what you do: get a fingernail file (or some other fairly pointy object) and gently lift up the edge of the nail that's hurting... just a little bit at first. Your eyes may water and it will be painful. Next, apply a gentle squeezing pressure toward the area or corner of the nailbed that you just lifted. You may have to give it a good, hard squeeze... and "milk" the area toward the corner you lifted. Hopefully, you'll have a bunch of yucky, zit juice-looking sauce erupt from the area. If this happens, consider yourself cured. If it does not (you don't see the sauce), do the procedure again. I had to do it three times on my middle finger. I just KNEW something was up in there that needed to come out! Sure enough, when I pushed my metal fingernail file a little bit deeper and a little bit deeper under the nail, out came the sauce. Gross, but it didn't cost me a dime. Now, once you do get the initial sauce out, repeat the procedure a few hours later or the next day -- because you will get more out. Repeat until it feels a bit better. Eventually the sauce will subside and it may just be blood/serous (clear) fluid. At this point, don't go diggin' up in there any more - let it heal up. Most people don't need to see a podiatrist for this.

Podiatrist...They will give you an injection and numb the toe so they can cut the side out to relieve the pressure and possible infection. They will lead to infections if not taken care of properly and will turn all kinds of lovely shades or purple. If you go to the dr, there are two possibilities. They would numb ya up and clip the whole side or it, or if its the beginning stages of it they may be able to just clip it out.
Soaking will only help if you are going to do it yourself, which is not suggested. If there is any sign of infection soaking will not do a thing!

Soak for a bit and then clip with clippers (take care only to clip the DEAD part of the skin). Are you quite certain it is a HANGNAIL which is causing your pain?

This is probably an ingrown toenail. you need to see a Podiatrist, as they have the correct tools to cut this out so the pain stops. Anything done at home will not last and work as good as a Podiatrist.
