We are a very small company ( 3 People ). We are looking for a program that can used online that will manage our clients. That way if someone goes online and makes a change to a client, the next person online will see the changes. We just need to manage the names, numbers, notes and maybe a couple custom fields. Anyone have any good advice that doesn't cost a fortune.

There are likely many solutions, but one we use for our group of near 200 members is gmail. It allows gathering of all the group's membership in a contacts file. You can add custom fields (e.g. in your case "notes") to a contact. More than one person can be logged on to a gmail userid at the same time, no problem. All this is free. The same gmail account can also be set up with a number of other free services, such as shared calendar, documents/spreadsheets, and other stuff...
