ground beef is cow meat (same as steak) that is ground up and processed. It is healthier if is is extra lean because it comes from less fatty parts of the cow. The leaner muscles to be exact.

It's usually pure beef, with or without some fat.
There is a machine, meat grinder. It has "funnel like" top where the meat is fed into it. Inside a shaft from the funnel is a worm gear. At the end of the shaft is a cutting blade, about four blades, much like a star. In front of the cutting blade a plate is attached with holes in it. As the worm gear feeds the meat through the shaft the blade cuts it and then the worm feed forces the cut meat through the plate with the holes.
The ground meat emerges like several long strings of yarn, although it is loose.
Link sausage is made the same way, although extending from the plate with holes, a tube is attached, on which natural intestine of pork or plastic tubing, called casing, is slipped onto. Then as the sausage is fed through the tube, into the casing, one must help the casing off the tube as it fills.
Linking is done by squeezing the casing at the desired length, then twisting the casing to form a link.
Feeding the meat into the funnel top can be dangerous. It must be pushed with a short round wooden pole. If one uses hands to do this, the worm may catch the fingers and hand and the damned machine, attached to the hand goes to surgery. Usually the hand is lost.

It depends on where you get it. If you go directly to a butcher shop, they'll ground it from just about any cut you want and they usually won't sneak in "filler", like gristle, extra fat, bone, etc.. Supermarket ground beef can have just about anything in it. The leaner the beef, the less fat, but also the less flavor.
Word of advice: NEVER buy the ground beef in those "chub" packages where you can't actually SEE the meat inside.

It's steak that's been put through a meat grinder, generally chuck steak I believe.
As for healthy if extra lean, all red meat should be in moderation anyway. Your body does need fat to function, and fat is what gives beef its flavor. Large chunks of fat are not healthy, but I usually like to use 80% lean meat. Anything lower is too fatty, but anything higher than 80% (or 85%) doesn't cook as well and doesn't taste as good.
Lots of fat will affect your cholesterol levels, but if you're just talking a burger now and then, don't worry. If you eat them every day for lunch, then you need to pay more attention to their fat level.
Frankly things like hot dogs and sausages are much worse for you.

As the name implies, it's beef that's been run through a meat grinder. The debate of healthy red meat rages on, but I say anything in moderation. The leaner the beef, the less fat your body has to process, so in that way, it's better.
