It's a figure of speech. In older times, the liver was thought to be the seat of courage; the idea of a 'lily' (white) liver was a metaphor for cowardice. Why this would apply to nymphos I have not a clue. Update: Apparently 'white liver' is a modern slang term; it's not clear where it comes from but I'd suspect the idea is a liver full of semen. It's still a metaphorical term, not a literal one.

I never heard of it as well, I do know that when a persons liver gets really bad, such is with hepatic cancer their feces will look like white clay. But I 'm pretty sure that is not what you are looking for as an answer.
