Should I submit a resignation letter or should I just call my employer and tell them that I'm not returning? Also, if I should submit a letter, what should it say? Should it be worded like a "traditional" resignation? Thanks!

Call your boss and tell him/her first. Follow up with a traditional resignation letter, letting them know that you will not be returning from leave, you appreciate everything you learned while you were there, etc.
