Do you mean they need glasses to get to 20/20 or that even with glasses they can't get to 20/20?

Needing glasses is due to a mismatch between the various optical components of the eye. Either the cornea grows a little too flat or too curved, or the lens inside the eye is a little too flat or too curved or the total length of the eye is a little too long or too short to work with the other elements. This is primarily a genetic factor, just like your nose shape or ear size. Generally, most people's need for glasses stabilizes between age 16 and 25 and they stop needing stronger and stronger glasses.

People who cannot achieve 20/20 even with glasses have had and eye injury or eye disease that interferes with vision. There are hundreds of different eye diseases with many different mechanisms for causing the vision loss, many different rates of progression and many different factors controlling the change.

During adolescence, when you grow a lot, your eyes often loose their shape, causing your sight to get worse. Sometimes they recover when you stop growing. Older people's vision deteriorates, just like every other part of them. Basically, we are living longer than our bodies are intended to live. In the wild, elderly people would have died as they lost their senses. Thankfully, we have good medical technology, and no lions will eat us!
