4 hours is the max time in a crate during the daytime for a dog. He needs out more often. You are expecting too much.

We have a male puppy as well. I am not sure what type of dog you have. We own miniature dachshunds and if we are gone a long time they do the same. It makes a big mess on the toys and the blankets in the crate. We have resorted to using diapers. We buy the pampers 18-28 lb size since our dog weighs about 12 lbs. We wrap it around there belly area. Attach it on the top or back area. It does not cover them if the poop. Yet, we have not had a problem with that. When we come home we take off the diaper and let them out. This has worked great. No more mess to clean up we just toss the diaper in the garbage.

If you've taken him to a vet and no urinary infection was found, and no worms were found, then you may want to get a second opinion.
Possibly he has a small bladder or poor bladder control. If you have a vet school nearby, ask your vet for a referral to the vet school, as they often see animals ONLY by referral.
Do you have some way you can put him on a runner outside where he has shade and a dog house to go into? so he could pee and then go into a shaded area?
How about a kid you could hire to take him outside for a walk so he'd have a break at the right time???

5-6 hours is too long to be leaving him in the crate. You will either have to go home at lunch or get someone to go by during the day to let him out. Dogs do not like to pee where they sleep and if the vet has ruled out a physical problem then you know you are leaving him in there too long.

He won't dehydrate if you restrict water intake while you're not home. It's pretty simple. What goes in must come out. Try a food/water schedule that fits you and the dog will adjust. For example, non water after 8:00 PM until you return home. If you don't restrict water, you'll have to be a bit more patient while his organs continue to grow and provide him better control/holding ability.

They say that once they urinate or poop in their crates they always will, but I haven't found that to always be true. How big is the crate? A crate that is too big could be part of the problem, if he can use one end as a bathroom and the other to lay down, then it's too big.

If you plan to leave him alone, take a newspaper and place some of his or another dog's urine on it. We he sniffs the paper later on, he should start using the paper to do his business or mark his territory. Of course a lot of praise comes with his new experience after he has learned what to do. I would also take him outside often when I am home and then of course a lot of praise again when he does his business outside.

well when he is that old their bladder cant hold that much they need to be taken out almost every 2 hr. and you must not have set a timeline to got out when he was younger. so the best thing is to on your lunch brake try to run home and take him out make a timeline its not to late to make one and on errands dont stay out longer than 3 hr. or just get a dog sitter.

Try a smaller crate and do not give them food or water right before putting him it.
