Lucky you! Jennifer has the right idea of how to attach a comb or two combs to it.
I had a purchased tiara and veil, and what my hairdresser did was genius (I guess its just the years and years of experience doing brides...), and I've passed this along to MANY brides. My tiara had one comb; my hair is fine and straight - though I had some light wave left in it from a perm, my hair was just curled with a curling iron, it was maybe 4-5 inches long on top.
Now what she did was strange, but it WORKED. Here she had taken all this time curling and making my hair beautiful, then she did this - in her hand, she just gathered up a fair sized hank of my hair from the middle top (making sure my bangs and sides were left nice), and she twisted HARD over and over, til it made a knot, basically, because it coiled up on itself. Then she jammed in pins from different directions (now that hurt!), and sprayed it.. Then the comb from the tiara slid in under that, and that sucker DID NOT MOVE all day and all night. And it was comfortable!
If you area attaching two smaller combs, one on each side, that might work with making smaller 'knots' on each side of your head.
Alternatively, try this 'knot' thing with the hairpins, and the loops already existing on the tiara as you have it.

Wire - not glue - a couple of clear combs to the tiara using thin, (22 gauge or so) silver wire. That way you can just remove the combs after the wedding and your tiara won't have been harmed.
Before you put the tiara on - take some craft spray glue - it's like spray-on rubber cement - so be careful - and spray JUST the combs (cover everything else) with the glue. You don't need much - just a spritz - which should help your tiara remain in place in your hair.
Also, before you put the tiara on - have the stylist spray your hair with a strong-hold hair spray. That will keep you hair in place for the combs to grab onto.
Between the combs, the spray glue and the super strong hairspray, this should keep your tiara on for the ceremony and pictures.

If trying to attach the combs doesn't work (great idea, btw!) take it to a jeweler that specializes in custom work. He'll be able to look at it and give you some thoughts. My only other idea would be to attach a thin line of velcro under the tiara. The velcro loops would take hold of your hair (it'll make it painful to remove but it may work) and most likely not budge.
You mentioned that this tiara is quite heavy, so you may want to re-think the idea of wearing it all day and evening. It's an almost guarantee of a headache and neck pain that you don't want to have on your wedding day. If you plan to wear it for the ceremony and pictures, think of having an alternate plan for the reception. Another thought - since it is sentimental to you I am assuming it is a family heirloom - maybe have it cleaned well to sparkle like crazy and put it under a glass dome on your cake table. For fun use a tiara theme for the reception. Have small ones (like the kind you can get at a novelty store) incorporated in to the centerpieces, see if your cake baker can make the cake top in the shape of one....there are all kinds of fun things you can do with it and this way you can display the heavy one and wear a lighter one.

what about attaching an elastic to two opposite ends and using that to offer some leverage for it to stay. (do not put in under your chin like a party hat, put it behind your head and under the hair do. The only other thing I can suggest that you probably shouldnt do....Celine Dion had her heavy tiara/with veil sewn into her head or hair... I forget which. But I don't suggest you do that.
Maybe you could bring this tiara to an anitque repair place and see if they can weld on some silver plated combs? Or do what the poster above said...that should work!

find some small clear hair combs (2), then attach them to the tiara by wrapping clear string (fishing line works great)...you will need to wrap it through each tooth of the comb and do it back and forth from one side of the comb to the other at least a couple of times to make sure it is held tight. then tie it off, but make sure you use a good knot, and if you can find some jewlery cement and put a dot on the knot this will help to keep the knot from slipping out. You could use jewlery wire and it will be stiff but I am afraid that it will have ends that you will have to deal with, but if you can find like beading wire or beading string at a craft supply house this will help.
Also when styling your hair you will want to make sure that you spray it with hairspray so that it is "dirty" before you do it because if you use soft silky hair the combs and hair pins from the tiara will slide out, which I am sure your hairstylist will know.

I wore a tiara for my wedding and my hair dresser teased my hair under my tiara to make my hair kinda tangled.... I am not sure how your wearing your hair, I wore mine half up and half down and I wore a veil also, so she got the part of my hair that was going up and teased the heck out of it under and then stuck the bobby pins in with the tiara and it stayed all night..... I had no worries, so try that.... try to get your hair just really think right there. Good Luck.

Try attaching a small hair comb to each side by winding thin metal wire around the tiara and the top of the hair comb. I made a tiara for my friends wedding and that is how i attached the combs. It stayed in place. I found the wire at Joann Fabrics.

If you aren't strict about keeping the tiara as is, maybe you could glue some teeth on it to help it stay put. Other than that, I'm out of ideas.
You could always sew it onto your head, a la Celine Dion.
