All food items have expiration dates. Your wine vinegar may seem to be fine, but as time goes on, it's quality and flavor may have diminished.

I wouldn't risk product that's been open for that long. I recommend replacing it with a new bottle.

Saving a few a dollars is not worth any health risks.

It's not likely a health risk because of the acid in the vinegar killing bacteria. It may not taste the best however. If it was white vinegar and now it is brownish, I would just dump it out. You can usually tell if there is a little brownish " dust" in the bottom of the bottle. That is known as "mother of vinegar" and lets you know that it is really probably at the end of its useful life.

Technically no, it doesn't. However, it will likely taste 'off' especially if there are a lot of basil leaves. Flavored vinegar doesn't last long after the demise of the leaves.

Generic: Keep tightly closed. Cloudy appearance doesn’t affect quality. Distilled vinegar keeps longer than cider vinegar

Myself I wouldn't use it.
