Having more in common with the waitress than the girl I was sitting across from! She had no brain, no humor, and no idea how to dress. I could go on, but I am trying to forget.

This is a good question. Obviously, the most awkward thing about a blind date is when the person is much less attractive than you would like them to be. I mean, let's face it: physicality plays a big part in whether or not we are going to be attracted to another person. It's not being shallow. It's biology. Natural selection. Our own, involuntary external marketing scheme to get us on track to a relationship. Animals do it to impress each other during mating season, plants use the same schemes for pollination (I'm personifying plants, but just flow with me), and people do the same thing. So when you get all dolled up and put on some smellgood for the blind date, your expectations are up because your friend has told you about how "nice" he is, or how "smart" she is, and "funny, witty, blah blah blah, yackitty-smackitty" they are and you roll up and you see homeboy with missing teeth/gold grill or miss thang with a moustace/twenty facial piercings and twelve tattoos (or whatever it is you find unattractive), your dreams are dashed and you feel a little cheated.
Other than that, the second most irritating thing about a blind date is when the person looks good, and has the personality of a parking meter. Zero conversation, no shared interests, gigantic ego, all sum up to a compatibility score of nilch.
Blind dating is such a bad idea. It should be outlawed.

Never had a blind date myself, but a friend of mine got hooked up on one. It turns out it was with an ex girlfriend of his! So it was a really short date.

When you meet in a really cool (mutually agreed upon) environment and he just is not speaking, or mumbling. I hate really conversationally challenged guys. Then the date ends and you think it was really bad... and they call and want to go out again?!?!? What's up with that ?

1. When they don't talk at all and that makes it awkward
2. them checking out other women
3. them getting really serious after only one date (creeps me out)
4. them expecting you to have sex on the first date (how rude!)
