It means to accomplish something just before a dead-line. Refers to the numbers on a clock obviously, and 11th hour refers to the last hour before the 12th hour.

It is something along those lines. If something happens and you are "saved" at the "11th hour" it basically is saying that if it hadn't happened you'd have had to give up or surrender.
