Well you need to get the smell out of that spot to keep him from going back to it. As long as he can smell it he will keep returning to that spot. I would suggest putting something there after you clean it like a piece of furniture or something so that he can't go back there.

If his litter box is always clean then maybe he might have a health issue. Maybe he has a urinary infection or something. I would take him to the vet to rule out any health issues.

He could be doing this because he can still smell the area where he soiled it. I would suggest using the product O-Ban. It is sort of like Fabreeze, and Wal-Mart sells it. It is usually on the isle where the Fabreeze is. It should get the smell out. And also try covering the area with a rug. That may also discourage the behavior.

You need to get some special shampoo for the carpet because that one spot has his scent on it so it is his territory now & his personal toilet spot so as long as his scent is gone he won't use that spot anymore! Good Luck x x x x x x x x x x

are you cleaning the spot well enough so that the scent is gone? if you clean it, and then use white vinegar on the spot, the vinegar gets rid of the scent. since it is only that one spot, i am assuming its a scent thing there. are you sure its only that spot and you just dont know about others? i have ones that will do that and only thing i can figure out is that i used to let them go outside and now im not, and now they are mad.

My advice is: Get a newspaper and hit the newspaper next to him but don't hit him as it will not solve the problem; cats get terrified from the newspaper
If this doesn't work, try spraying drops of water on him

When cats are having a urinary tract infection, it is painful and they often associate the litter box with pain, so they avoid it. Have your vet check him out and test his urine. If it normal, then he/she can discuss behavior options with you. Here is a good website discussing this:
