With current screens, not much, but with older CRT (television-like screens) a constant image did a sort of "burn" into the screen, if they were left on long enough, so you'd see the words (or whatever) all the time. A screen saver moved the image around so nothing had a chance to really "burn in", and thus "saved the screen".
You can still sometimes see this on ATMs that don't have screen savers - they have login information permanently displayed.

On original crt monitors the screen would eventually burn the layout of a page in if it was left on that page regularly. The idea was to make the screen do something different if the machine was unused for a while. I know one case where a similar problem was encountered in another way. A CCTV system using crt monitors (at a prison) had been running for about 18 months. A new external patrol system was set up, and a guard came back from patrol and saw a van parked near the wall which he had not seen on patrol. He went back to check and it wasn't there. It turned out to be a van which parked there every night, and it was still visible in the daytime. The cure for this was to make the cameras move a small amount at intervals.

With old CRT monitors (most use LCD now) it would burn an image into the glass if you left it there too long. This still happens somtimes with LCD called Image Persistance. With a screen saver this is less likely to be a problem.

it stops the pics or visual from burning into your screen, and the moron that said with lcds it does not matter, he/she is not nery bright, LCDS are the worst for having the backgrounds burning into the screen if left idle and sitting there for any period of time, the screen saver keeeps all the dots per inch from sticking and staying in 1 spot, that does not mean you need a very authentic screensaver for that will slow your computer down drasticallly! good luck and do use one or eventually you will see 1st hand what i'm talking about!

Well it used to help prevent images from burning into your computer's screen but nowadays it's more or less just a customizable feature.

I use it to display my photos or look cool.
Otherwise I set my system to go into power save mode and blank the screen to save energy.
