Jelly only uses the juice of the fruit. Jam uses the juice plus the pulp. The pulp is puree'd, so it's still smooth. If you left the pulp whole, or in chunks, you would have what they call "preserves".
It you want to make it even more confusing, there's also "marmelade", which is like preserves, but they also use the fruit peel as well.

Jam is softer than Jelly. Jam has more wholesome fruit puree than Jelly, jelly is made from the concentrated juice.

Jelly is clear sort of, Jam has pieces of fruit in it. It's not strained like jelly is and I think jelly is more fruit juice and jam is more fruit itself.

Jams normally have fruit in them.
Jelly is normally the liquid gelled.
I think the name varies from country to country and the lines have become blurred.
