My guess is that it burned. Have you ever eaten a burnt peanut? It's awful. I've never just had peanut butter over heat in a pan. I have used it as an ingredient in sauces, I usually leave it out till nearly the end of the cooking and just let it warm through.
Since it was a jar and not a pot, I think it rotted. I can't think of a reason to add water to peanut butter in the jar and leave it. The salt and oil preserve it, the water breaks down the preservatives and it goes bad.
Found a peanut....

hmm, sounds like it started fermenting. water isn't exactly pure, it does have other stuff in it besides h2o, like nitrogen. sooo it seems like the bacteria in the peanut butter was having a feast with the water u poored in the pot. as for the smell, that would basically be the waste that the bacteria excreted. kinda gross, i know, but everything is covered and is made up of bacteria, including peanut butter. so yea, basically, when u poored water in with the peanut butter, the bacteria within the peanut butter was presented with food and began to eat. of course, they had to excrete waste, cuz it's totally natural, causing that smell.

Here is a good peanut butter treat, melt a bowl of peanut butter in the microwave for about 10 sec, and pour the butter over ice cream to make it harden- it's tasty!
(Don't add water)

Perhaps it is not the water mixed with the PB at all, you may be allergic to peanut butter.
