Yes, the FAA requires all flight attendants and pilots to carry their passports when they fly.
Here is another reason that the other people may have not brought up. There is a system in the airline world called CASS. It's basically a program that tells you if a pilot or flight attendant is authorized to ride in the jumpseats in the flightdeck.
In order to gain access to the flightdeck, the pilot/attendnat has to have their passport and airline ID run through the system to make sure they are allowed to ride up front. They may have to use it when there are no seats up front for them to sit.

Yes all flight crew are required to carry their passport at all time along with other things like their manuals completely updated at all times because the FAA ( Federal Aviation Administration) do random checks and you could be fined for not having your required items.

Yes they do, they are entering another country. If some countries require a visa then the pilots are required to also have one.

yeah... anybody who goes in and out of the country needs a passport.... including flight crews.
