As long as he graduated, he will be called a newly graduated doctor. After graduation he will go through internship, so he will called a medical doctor "intern". Going to the next step he will have the residency training so he will be called a resident doctor.

In my year, they would have elected him president of Zeta Omega Zeta, the facetious opposite of AOA the fraternity of the top ten percent of medical schools. Some the best people-friendly doctors are not high achievers. (Any US grad would have little trouble getting a residency slot.)

might be hard for him to find residency. but beyond that, it doesn't matter. schools over. nobody will ever look at your grades again.
