Go to the PD and tell them what has taken place and ask them what is required of you to do so

Where was the sim card stolen? You have to make a report in the jurisdiction where the theft occured. You may be able to make the report over the phone if it is out of state. Some departments will do this others will not. Call them and inquire. They may also require you to go the police department that has jurisdiction over the town that you reside in to file a report there. Make sure you cancel your cell service. Call your local PD and inquire about their protocol for such cases.

If the Sim card and/or phone was stolen in your city, report it as a theft to your local police department. Give them the phone records to help them locate the suspect. They most likely will work with the PD in the area where the phone is now located.

If the theft happened elsewhere, report it to the PD where the theft occurred.

Contact the local agency (sheriff's dept. or police) some agencies take reports over the phone. Have you cancelled service with your provider?
