Is it safe to eat when it gets that way? Is there any way to avoid this?

According to the USDA:

"Black speck is a disorder where 0.5 to 4 mm (0.02 to 0.16 in) diameter necrotic lesions appear on the surface of branches or flower stalks in the interior of the curds. This disorder is more prevalent on certain cultivars and is most severe in cauliflower produced under periods of warm weather and rapid growth (Loughton and Riekels, 1988). Boron deficiency can result in brownish discoloration of the curd and pith of the stems and may result in hollow stems. In addition, blisters and cracks may form on the midribs of leaves and curds may taste bitter (Dearborn and Raleigh, 1935).

Postharvest Pathology: The major causes of postharvest decay are bacterial soft rot caused by Erwinia and Pseudomonas spp., and brown rot caused by Alternaria spp. (Ceponis et al., 1987). Storing only good quality, disease-free heads and maintaining good temperature control can best control these decay organisms."

Cut off the brown spots, wash the head of cauliflower, shake off excess water, wrap in a paper towel, enclose in plastic bag and put in refrigerator. It will keep a while with some further cutting possibly necessary. I use a food saver to vacuum pack my veggies in hard plastic containers and everything keeps for 2-3 weeks, if not eaten quickly. It's great.

Just cut it off. It's mildew. Won't kill you. But you've buying pretty old heads of cauliflower if you get it all the time. Complain to hte grocery store about freshness.

I cut that stuff off. I love fresh cauliflower and hate the brown stuff.

Fresh cauliflower doesn't keep that long. Buy smaller amounts more often.

If your storeing it in a place that is warm that is what happens to it.keep it in the fridge..also you can cut the bits of and eat it..make sure you cook it fist it wont hurt you as i have done it myself..
