Not only do you need Anti-Virus program, Anti-Spyware and Firewall, but you also need a good Anti-Rootkit as well.
Rootkits are used to hide the presence of a malicious object like trojans or keyloggers on your computer. If a threat uses rootkit technology to hide itself it is very hard to find the malware on your PC.

TIP: The apps above can only clean up so much.
As a last resort, back up all personal data from your PC (copy to CD / DVD / external hard drive) and do a Windows "Clean Install". For instructions, go to http://www.theeldergeek.com
Note: You must use your Recovery disks, if included with your PC, to avoid driver problems, not a store-bought XP. The Recovery disks may be ON the hard drive, so Read The Friendly Manual FIRST.

SpywareBlaster runs alongside Spybot nicely. It will stop a lot of the spyware intrusions totally. And it's free!

The only things you need are the updates. This should protect you quite well. The only other thing you might consider but is not required is a firewall from zonealarm.
