First find out why it hurts, he probably won't know. Take some medicine and call the doctor. The doctor will probably order an xray to examine the bone and the hardware in the ankle making sure nothing has moved or shifted.
It has to be examined by the doctor

Have him soak his foot and ankle in hot water with 1c. of dissolved Epsom salts. Afterwards massage an analgesic cream. do this twice daily. Advil soft gel capsules work quickly to relieve pain. Have him take them four times daily. If after two weeks he does not feel better have him see an orthopedic doctor for an examination and treatment.

I suspect arthritis has developed in his ankle. This is quite common in old joint injuries and surgeries. Have him try treating it as such to see if it helps alleviate some of his discomfort. Regardless, he should still see a doctor for a diagnosis. There could be a more serious problem.

Tylenol and elevation, maybe some heat. It will always make its presence known. If it is that bad he should see his doctor.
