Every time it comes to a complete stop (in drive) it revs at about 500rpm and shakes a really lot. If it is in neutral it doesn't though. The engine has over 146,000 miles. Any idea what might be causing it? Does the engine need to be rebuilt? Btw the transmission was rebuilt awhile ago.

The spark plugs, wires, and head gaskets have all been replaced.

check for vacume leak, check the speed idle motor. or the TBS.

Sounds like the motor mounts are shot.

That is about the only thing it could be while stopped.

** Try this :

Open the hood, and press the brake pedal.
Now shift from reverse to drive and watch the engine, there should be very little movement, if more than 1" I would get new mounts

try the plug wires.. and the map sensor. test the map by using a vaccuum pump or a hose and your breath, testing to see if it will hold pressure or vaccuum. and while the engine is running raise your hood in the dark, and see if the plug wires spark, that would be leakage and time to replace them.

no its not the engine relativly cars alway have to be at 1000 rpm when idel that means the odometer must be on 1 when car is in drive can be two things simply need to be put back into time any mechanic can do this with a light gun or worst needs a whole tune up spark plugs, wires, oil change, pcv, oil filter and then put back in time basically your idel is to low thats why it shacks like if it was going to turn off.

Possibly the motor mounts. If you're in neutral the trans is absorbing some of that lateral motion.
